Introduction: Lower gastrointestinal bleeding (LGIB) poses a considerable complication in colorectal cancer (CRC) patients. This study elucidates the incidence, outcomes and complications related to LGIB in CRC patients based on data from the 2018 Nationwide Readmissions Database (NRD).

Methods: CRC patients experiencing LGIB were identified from the NRD 2018 database using specific ICD-10 codes. This extraction consisted of patient demographic data, clinical attributes, co-existing conditions, and treatment particulars. The derived data was evaluated through descriptive statistics, chi-squared tests and kaplan meier survival curve.

Results: The study population consisted of 58,569 unique patients diagnosed with CRC, of which 12.5% (7,334) experienced LGIB during their hospital stay. Mean age of these patients was 70.7 years (compared to 68.0 without LGIB), with a majority being male (55.1%). The mortality rate for the initial admission was 4.0%. Survivors from the initial admission faced a 25.5% risk of readmission within 30 days (KM curve), with the mortality rate increasing to 5.1% during readmissions (p< 0.01). Palliative care encounters constituted 10.4% of initial admissions, where mortality was significantly higher at 23.4% as opposed to 2.1% (p< 0.01) in non-palliative encounters. Detailed outcomes of initial and readmission instances are outlined in Table 1. There was no significant difference in rate of readmission among patients who underwent colonoscopy (CSP) during index admission (26.1% with CSP vs 25.4% without, P=0.1035). Mortality was lower for patients undergoing CSP (2.6% vs 5.5%, P< 0.01).

Discussion: This study illuminates the substantial incidence of LGIB among CRC patients and its influence on patient outcomes, highlighting the need for effective LGIB management strategies. A notable proportion of CRC patients experienced readmission within 30 days, suggesting the necessity for improved post-discharge care and patient management plans. The significantly higher mortality rates in palliative care encounters underscore the importance of palliative care in enhancing the quality of life for severely ill patients. Future research should focus on interventions to reduce LGIB and readmissions in CRC patients, and examine the role of palliative care in patient outcomes.
