P0414 - Barrett's Esophagus in the Asian Population: A National Cohort Study

Introduction: Barrett esophagus (BE) is a precancerous finding that can progress to esophageal cancer without proper intervention. It has been reported that the prevalence of BE is significantly lower in Asian countries. However, there is a lack of data on the Asian population in the US. In this study, we analyzed the prevalence and characteristics of Asian Americans with BE.

Methods: We conducted a retrospective study utilizing the 2019 National Inpatient Sample (NIS) database and ICD-10 codes to identify patients with BE and BE-associated comorbidities. The patients were stratified into BE in the Asian and non-Asian to compare their characteristics. A multivariate logistic regression model with covariates including age, gender, and BE-associated comorbidities was adopted.

Results: The 2019 NIS sample consisted of 128320 patients with BE, including 1320 Asians, which showed a significantly lower prevalence (0.12% vs 0.37%, P < 0.001). Asian patients with BE had older age (70.82 vs 67.97, P = 0.001), a higher mortality rate (3.79% vs 1.52%, P = 0.004), and higher hospital-associated costs (85822.05 vs 61764.83, P = 0.004) than non-Asian patients. Asian patients demonstrated a significantly lower prevalence of GERD (46.21% vs 59.87%, P < 0.001), obesity (8.33% vs 18.96%, P < 0.001), and alcohol-related disorders (4.55% vs 9.30%, P = 0.010). Nevertheless, Asians with BE manifested a high rate of DM (39.39% vs 29.04%, P < 0.001). 2019 NIS contained 885 Asian esophageal cancer cases, yet none was diagnosed with BE. In a multivariant logistic regression in which gender, age, and comorbidities (including alcohol-related disorders, GERD, H. Pylori infection, obesity, and DM) were adjusted for, Asian patients with a history of GERD (OR = 4.683, P < 0.001), a history of alcohol-related disorder (OR = 2.690, P = 0.002), male gender (OR = 1.887, P < 0.001), and older age (OR = 1.040, P< 0.001) had higher rates of BE.

Discussion: Our result indicated that BE prevalence is remarkably lower among the Asian population, which is compatible with the published studies. Compared to the non-Asian population, Asian patients with BE were older and had a higher mortality rate. Based on the comparison of BE in Asian and non-Asian patients’ characteristics and the multivariant logistic regression of Asian patients, a history of GERD and alcohol-related disorders are the two most dominant factors associated with BE bedsides gender and age.
