Introduction: Colorectal cancer (CRC) is an important cause of morbidity and mortality worldwide. Due to large different demographics among the different states of the United States (US), we aim to determine trends in the prevalence, incidence, mortality, and disability-adjusted life years (DALYs) in the US and across different states and genders.

Methods: Using the Global Burden of Disease database, we analyzed the prevalence, incidence, mortality, and DALYs in the US between 1990 and 2019. We computed relative percentage changes (RPC) over the study period and compared these between states.

Results: The age-standardized prevalence of CRC in the US has decreased from 273 per 100,000 population in 1990 to 259 per 100,000 population in 2019. The CRC prevalence has decreased in the US over the study period (age-standardized RPC=-5.2%). Massachusetts, Minnesota, and Wisconsin had the greatest decrease in prevalence (-27.3%, -20.3%, and -18.0%) among all states. The incidence of CRC decreased in the US over the 30 years from 47.6 per 100,000 population in 1990 to 41.9 per 100,000 population in 2019 (RPC=-12%). Massachusetts, Minnesota, and Rhode Island had the greatest decrease in incidence (-32.9%, -24.1%, and -23.9%). The decrease in incidence rate for CRC was higher in males (-14.1%) compared to females (-12.6%). The mortality rates decreased in the US from 20.0 per 100,000 population in 1990 to 14.8 per 100,000 population in 2019 (RPC -26%). The southern states had the highest mortality rate for CRC in 2019, with Mississippi having the highest rate of 20.1 cases per 100,000 population in 2019. Massachusetts, New York, and the District of Colombia had the greatest decrease in mortality over the study period (-42.1%, -41.4%, and -40.9%). The decreased mortality was found in all states except Mississippi, where the mortality of CRC increased over the study period (+1.5%). In the US, age-standardized DALYs decreased for both genders, with a larger decrease for females than males (24.7% vs. 23.8%) for the past 30 years.

Discussion: Advancements in CRC treatment have led to decreased incidence and mortality rates in the US. This is likely due to endoscopic interventions and screening uptake. Southern states with higher rates of obesity, tobacco use, and limited physical activity show increased incidence and mortality rates, but advancements in treatment have improved outcomes nationwide.
